Indications:
1 Replenish energy and body fluids; It is used for insufficient food or large body fluid loss (such as vomiting, diarrhea, etc.) caused by various reasons, total intravenous nutrition, starvation ketosis. 2 Hypoglycemia; 3 Hyperkalemia; 4. Hypertonic solution is used as tissue dehydrating agent; 5. Preparation of peritoneal dialysate; 6 Drug diluents; 7 Intravenous glucose tolerance test; 8 For the preparation of GIK (polarized liquid) liquid.
Usage and Dosage:
1 When patients with caloric energy supplementation have reduced or cannot eat for some reason, they can generally be given intravenous injection of 25% glucose injection, and at the same time replenish body fluids. The amount of glucose used is calculated based on the amount of heat required.
2 Total Intravenous Nutrition Therapy: Glucose is the most important energy source for this therapy. In non-protein caloric energy, the ratio of glucose to fat is 2:1. The dosage depends on clinical caloric needs. According to the needs of the amount of rehydration, glucose can be prepared to 25%~50% of different concentrations, if necessary, add insulin, add 1 unit of regular insulin per 5~10g of glucose. Due to the normal application of hypertonic glucose solution, it is more irritating to the veins and the infusion of fat emulsion is required, so large intravenous infusion is generally used.
3) For hypoglycemia, severe cases can be given intravenous bolus injection of 20~40ml of 50% glucose injection.
4. Starvation ketosis, severe cases should be treated with intravenous infusion of 5%~25% glucose injection, and 100g of glucose per day can basically control the disease.
5. Isotonic water loss is given intravenous infusion of 5% glucose injection.
6 Hyperkalemia should be treated with 10~25% injection, and 1 unit of regular insulin infusion per 2~4g of glucose can reduce serum potassium concentration. However, this therapy only allows extracellular potassium ions to enter the cells, and the total potassium content in the body remains unchanged. If potassium removal measures are not taken, there is still a possibility of recurrence of hyperkalemia.
7. Tissue dehydration hypertonic solution (generally 50% glucose injection) is rapidly injected intravenously with 20~50ml. But the effect was short-lived. Clinically, attention should be paid to preventing hyperglycemia, and it should be used sparingly at present. When used to adjust the osmotic pressure of peritoneal dialysate, 20ml of 50% glucose injection, that is, 10g of glucose can increase the osmotic pressure of 1L of peritoneal dialysate by 55mOsm/kgH2O.
