Action mechanism
The main pharmacological action of isosorbide nitrate is to relax vascular smooth muscle, and then to cause peripheral artery and vein dilation, especially for the latter. Venous dilation can promote peripheral blood accumulation and reduce venous return, and reduce ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Relaxation of small artery smooth muscle can reduce systemic vascular resistance, arterial systolic blood pressure and mean arterial pressure (afterload). Isosorbide nitrate also dilates coronary arteries.
